How to Apply: Follow these steps to request an AED program package to be donated to your organization.

1. 1. Review the eligibility criteria
2. 2. Complete the application form

AED Program Package includes:

• AED and carrying case - Philips HeartStart OnSite device
• AED and CPR on-site, one-time training for 2-year certification
• One year of medical direction and oversight (required in the District of Columbia)
• Registration of the device (required in the District of Columbia)
• Site assessment for placement of AED
• AED wall mount or wall rack
